Optum Quit For Life Mini Quit Challenge Promotion Rules

Promotion Sponsor: Optum Quit For Life and 1-800-QUIT-NOW sponsored by the New Mexico Department of Health – Tobacco Use Prevention and Control Program ("Sponsor"), 1190 S Saint Francis Dr Santa Fe, NM 87505.

The Entry Period for Mini Quit Challenges (the "Promotion") starts at 7:00 a.m. MT (Mountain Time) March 7, 2022, and continues until 11:59 p.m. MT March 3, 2024. The Promotion consists of a series of mini quit challenges (hereinafter referred to as the "Challenge" or "Challenges").

The Mini Quit Challenges will be held as follows:

Delay & Distract Mini Quit Challenge Must enter online or via text before 11:59 p.m. on April 2, 2023. Entrants must pledge to practice a smoking replacement action, such as chewing gum, every time they experience cravings on April 3, 2023 to be eligible to win. All eligible entrants will be entered into a drawing for a $100 Amazon Gift Card.

Smoke-Free Home Mini Quit Challenge Must enter online or via text before 11:59 p.m. on April 30, 2023. Entrants must pledge to not use tobacco in their home on May 1, 2023 to be eligible to win. All eligible entrants will be entered into a drawing for a $100 Amazon Gift Card.

BONUS: Car Mini Quit Challenge Must enter online or via text before 11:59 p.m. on May 14, 2023. Entrants must pledge to not use tobacco in their car for the entire day of May 15, 2023 to be eligible to win. All eligible entrants will be entered into a drawing for a $100 Amazon Gift Card.

Walk it off Mini Quit Challenge Must enter online or via text before 11:59 p.m. on June 4, 2023. Entrants must pledge to replace one cigarette break on June 5, 2023 with a walk or other healthy activity to be eligible to win. All eligible entrants will be entered into a drawing for a $100 Amazon Gift Card.

Personal Trigger Mini Quit Challenge Must enter online or via text before 11:59 p.m. on July 9, 2023. Entrants must pledge to identify and refrain from acting on one of their personal tobacco triggers by not using tobacco in response to their trigger on July 10, 2023 to be eligible to win. All eligible entrants will be entered into a drawing for a $100 Amazon Gift Card.

Just Say “No” Mini Quit Challenge Must enter online or via text before 11:59 p.m. on August 6, 2023. Entrants must pledge to say “no” to at least one tobacco craving on August 7, 2023 to be eligible to win. All eligible entrants will be entered into a drawing for a $100 Amazon Gift Card.

Change It Up Mini Quit Challenge Must enter online or via text before 11:59 p.m. on September 10, 2023. Entrants must pledge to not use tobacco where they would usually use tobacco on September 11, 2023 to be eligible to win. All eligible entrants will be entered into a drawing for a $100 Amazon Gift Card.

Social Smoking Mini Quit Challenge Must enter online or via text before 11:59 p.m. on October 8, 2023. Entrants must pledge to not use tobacco with friends or coworkers on October 9, 2023 to be eligible to win. All eligible entrants will be entered into a drawing for a $100 Amazon Gift Card.

Waking Up Mini Quit Challenge Must enter online or via text before 11:59 p.m. on November 5, 2023. Entrants must pledge to not use tobacco for two hours after waking up on November 6, 2023 to be eligible to win. All eligible entrants will be entered into a drawing for a $100 Amazon Gift Card.

Choose Your Own 2 Hours Mini Quit Challenge Must enter online or via text before 11:59 p.m. on December 3, 2023. Entrants must pledge to not use tobacco for two hours of their choosing on December 4, 2023 to be eligible to win. All eligible entrants will be entered into a drawing for a $100 Amazon Gift Card.

Smoke-Free Day Mini Quit Challenge Must enter online or via text before 11:59 p.m. on December 31, 2023. Entrants must pledge to not use tobacco from 12:00 a.m. to 11:59 p.m. on January 1, 2024 to be eligible to win. All eligible entrants will be entered into a drawing for a $100 Amazon Gift Card.

Car Mini Quit Challenge Must enter online or via text before 11:59 p.m. on February 4, 2024. Entrants must pledge to not use tobacco in their car for the entire day of February 5, 2024 to be eligible to win. All eligible entrants will be entered into a drawing for a $100 Amazon Gift Card.

Smoke-Free Meals Mini Quit Challenge Must enter online or via text before 11:59 p.m. on March 3, 2024. Entrants must pledge to not use tobacco for two hours after each meal on March 4, 2024 to be eligible to win. All eligible entrants will be entered into a drawing for a $100 Amazon Gift Card.

Terms for Challenges:

  1. Entry for each Challenge is no purchase necessary. Limit one entry per person/email address/mobile phone number regardless of method of entry. To receive notification of Mini Quit Challenge events and enter the Mini Quit Challenge drawings, text "Enter" to 1-855-965-4346 or visit miniquitchallenge.com to sign-up for email or text notifications and entry. You must enter individually for each individual Mini Quit Challenge. Entries for one challenge will not be held over for additional challenges. Entry by text constitutes your permission for Sponsor to send you text messages related to this Promotion. Reply STOP to cancel. Opting out from receiving messages will not affect entry. Please do not text and drive. Note: your mobile service provider may charge you for each text sent/received. Please check with your mobile service provider for pricing plan details. You are solely responsible for such wireless charges. Not all wireless carriers participate. By sending a text message, you give Sponsor consent to call or text your mobile phone/device should you be selected as a potential winner. You may not enter from another person’s mobile phone/device.
  2. Eligibility: Promotion is open to residents of New Mexico who are 18 years or older at the time of entry. Entrants must also be current tobacco users (cigarettes, cigars, pipes, chewing tobacco, smokeless tobacco, electronic nicotine delivery systems) at the time of entry. [Current employees of Sponsor, their immediate families and household members, and their respective advertising and promotional agencies are not eligible.] Promotion is void outside New Mexico and where prohibited, and subject to all applicable laws.
  3. Drawing and Awarding of Prizes: Random draws to select the potential winners from all eligible entries received for the Challenges will be conducted by a representative of the Promotion Sponsor within five (5) business days of conclusion of each Challenge in Minnesota. The first selected individual entrant will be eligible to win the corresponding Prize. Odds of winning any Prize depend on the total number of eligible entries received for each specific challenge. Potential winners will be contacted within thirty (30) calendar days of the selection date by email or text based on the contact information indicated on their entry. If a potential winner is not reached within 24 hours of being selected or if the selected entrant declines the Prize, or does not otherwise comply with these Promotion Rules, all related Prizes will be forfeited and returned, if necessary, and Sponsor may conduct another random draw in the same manner to select a replacement entrant. Potential winners of any Challenge may be requested to provide proof of identification and eligibility.
  4. Identification: If the identity of an online or text entrant is disputed, the authorized account holder of the email address or text services account submitted at the time of entry will be deemed to be the entrant. The individual assigned to the email address for the domain associated with the submitted email address or for the text number account used to enter is considered the authorized account holder. A potential winner may be required to provide proof that he/she is the authorized account holder associated with the selected entry. All electronic entries must be submitted from a valid and verifiable email account or text number, validity to be determined solely at Sponsor discretion.
  5. Liability/Publicity: Potential winners of any Prize may be required to sign an Affidavit of Eligibility and Liability/Publicity Release and IRS form W-9 within five (5) days of being notified before being named a winner, failing which they will forfeit their Prize. Winners agree that their name, likeness, city of residence, biographical information and/or any related materials and information may be used without limitation by Sponsor for publicity, promotional and/or advertising purposes (except where prohibited by law) in any medium throughout the world without additional compensation or right of approval.
  6. Prize Delivery: Delivery date of any Prize will be within 90 days from the time the Promotion Sponsor verifies the eligibility of the winner and receives any requested signed and valid Affidavit of Eligibility and Liability/Publicity Release and signed IRS W-9 form from the winner.
  7. Prize Substitutions/Limitations: Prizes must be accepted as awarded and may not be exchanged. No substitutions are allowed except at Promotion Sponsor’s sole discretion. The Promotion Sponsor reserves the right to substitute a prize for a substitute prize of at least equal value for whatever reason. Sponsor will award the prize to winner in a manner to be determined, based on location of winner and related logistical considerations. Any other costs beyond the value of the prize will be the sole responsibility of the winner, including taxes, unless otherwise agreed by Sponsor. All Amazon Prizes are subject to Amazon’s terms and gift code limitations.
  8. Sponsor Decisions Final: The Promotion Sponsor’s rulings are final and without appeal in all matters related to this Promotion and the awarding of the Prizes.
  9. Limitations on Liability: All entries become the property of the Promotion Sponsor and will not be returned. Sponsor is not responsible for any computer, online, telephone or technical malfunctions that may occur; for any incorrect or inaccurate information, whether caused by website users, equipment or programming associated with or utilized in the Promotion or any technical error which may occur in the processing of entries; for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to or alteration of entries; or for any problems, failures or technical malfunction of any telecommunications network or lines, phones, computer online systems, servers, providers, computer equipment, software, email, players or browsers on account of technical problems, traffic congestion on the Internet or at any website or due to any combination of the foregoing. Sponsor is not responsible for any injury or damage to an entrant’s or prospective entrant’s phone, computer or to any phone, device or computer related to or resulting from participating, attempting to participate or entering the Promotion; for theft, destruction, unauthorized access to or alteration of entries; or for problems downloading entries from any source. Sponsor assumes no responsibility for any incomplete, misdirected, late or lost entries, or theft or destruction or unauthorized access to, or alteration of, entries or entry data.
  10. Modification/Responsibility: Promotion Sponsor reserves the right to cancel, amend, modify or terminate this Promotion or the Rules at any time in its sole discretion and without notice. Any attempt to deliberately damage any website or to undermine the legitimate operation of this Promotion is a violation of criminal and civil laws. Should such an attempt be made, the Promotion Sponsor reserves the right to seek remedies and damages to the fullest extent permitted by law, including criminal prosecution. The Promotion Sponsor is not responsible for any errors or omissions in printing or advertising this Promotion.
  11. Entrant’s Rights: By entering this Promotion, entrants agree to release and hold harmless Sponsor and their respective employees, officers, directors, agents, representatives, successors, assigns and advertising and promotional agencies from any liability, claim, or cause of action for any loss or damage of any kind to the entrant or any other person in connection with this Promotion or participation in any Promotion-related activities, including but not limited to the use or misuse of a prize or any portion of a prize including personal injury, death or property damage. This Promotion will run in accordance with these Promotion Rules, subject to amendment by the Promotion Sponsor. Entrants must comply with these rules, and will be deemed to have received and understood the rules if they participate in the Promotion. Also, participants give their express permission to be contacted by Sponsor by telephone, email, mail, and text (if text communication is initiated by entrant) and agree to be bound by these rules.
  12. Facebook Role: This Promotion is in no way sponsored, endorsed, administered by or associated with Facebook. Entrants are providing their personal information to Sponsor, and not to Facebook. The information provided will only be used as indicated by the Sponsor, and entrants have the ability to opt-out of further communications.
  13. Dispute Resolution/Choice of Law: Issues concerning the construction, validity, interpretation and enforceability of these Official Rules shall be governed by the laws of the State of New Mexico. All disputes arising out of or connected with this Promotion will be resolved individually, and without resort to class action, exclusively by the appropriate federal or New Mexico State Court located in New Mexico. Should there be a conflict between the laws of the State of New Mexico and any other laws, the conflict will be resolved in favor of the laws of the State of New Mexico. All judgments or awards shall be limited to actual incurred out-of-pocket expenses (excluding attorneys' fees) associated with participation in this Promotion and shall not include any indirect, punitive, incidental and/or consequential damages.
